Prior to enrolling in a Field Placement, 咨询实习, or Internship in Diagnostic Assessment course, the appropriate application must be completed and submitted for approval. Complete application packets can be found in the Program Handbooks and Planners.

Field Placement Applications Must Include

1. Field Placement Request Form
2. Field Placement Preparation Form
3. Philosophy of 学校心理咨询
4. Acknowledgement of Handbook Review
5. Field Placement Pre-Hours Contract (if applicable)
6. Unofficial copy of Lindenwood Graduate Transcripts
7. Copy of background check clearance. Students should complete a Family Care Safety Registry (FCSR) Background Screening. The clearance letter must be dated no earlier than six months prior to the submission of the Field Placement Application Packet. Directions for completing a background check can be found in the 学校辅导手册.

Internship in Diagnostic Assessment Application Must Include

1. Internship in Diagnostic Assessment Approval Form
2. Internship in Diagnostic Assessment Preparation Form
